ELIGIBILITY CHECKLIST

FILING STEPS
- Choose an agent you trust completely — name an alternate in case the first cannot serve.
- Initial each specific power you want to grant in FORM 1 (or FORM 2 for limited use).
- Sign FORM 1 in front of a notary public; the agent must sign the Acknowledgment.
- Keep the original in a safe place; give a copy to your agent.
- If the POA involves real estate, record it with the County Chancery Clerk.
- Recording fees in Mississippi are typically ~$10–25.
- Provide copies to banks, investment firms, insurance companies, and other relevant parties.
- To revoke: complete FORM 3 and deliver to agent and all third parties immediately.
